Dr. Jennifer Leavitt (Family Physician, Mom, & Director of St. Paul’s Hospital Maternity Clinic) has developed a tool for helping pregnant people in the lower mainland of BC navigate social resources and programs available to support better access to SDOH. It can be used by pregnant people, their support people, health care team members, students.
